Getting started with Appalachian Mentoring Project is as simple as 1-2-3! Follow the instructions on this page to begin the application process.

Step 1: Download the Introduction to Appalachian Mentoring Program

Download the Intro to AMP by clicking the button, below. In this document, you’ll find details about the AMP Program, the Approval Process, and the Program Oversight.

Step 2: Complete the Online Application

Are you applying to be a Mentor or a Mentee? Click the corresponding button below to complete the Online Application Form.

Mentor Application

Would you prefer to Print and Mail an Application?

If so, download a copy of the Mentor Application or Mentee Application.

Then, send the completed form by:

Step 3: AMP will Follow-up

Once you have completed your application, you will be contacted by an AMP team member with further instructions.
